It would seem to be a systemic failure from policy across multi-agencies right down to the individual.
The bottom line must be that some people have not done their jobs correctly but whether that is from the bottom up or not we can only hope the inquiry will identify. I read that the inquiry's findings will not be public.
I wonder if it is one of those things that I seem to see daily taken to catastrophic extremes - people failing to use their initiative and take responsibility in their jobs.

I see the folks that failed to protect Baby P are being made to stand up and be counted. That report sounds very scathing.
